Fees and Policies

Insurance: Dr. Hall is not in-network with any health insurance. A receipt for out-of-network coverage can be requested. The patient can submit the receipt to their health insurance if they have out-of-network benefits.
​
Bills are payable by check, cash, health savings account (HSA), or credit card. Insurance is not accepted.
​
Rates: $300 for a one hour intake which consists of 45-50 minutes spent with client and 10-15 minutes doing paperwork. $150 for a thirty minute medication management which consists of 20-25 minutes spent with patient and 5-10 minutes doing paperwork.
​
Patients are preferred to be seen in the office but once stable will be given the opportunity for telehealth visits.
​
Pre-authorizations: $25
​
Controlled substances are prescribed at the prescriber’s discretion. The most common controlled substances are the ADHD medications, anxiety medications and sleep medications. Opiates, Suboxone and other narcotic drugs are not prescribed.
​
Copies can be requested and are subject to $.60 per page.
​
Missed appointment without 24-hour notice will be charged ½ the visit cost first time, 100% thereafter.
​
Court appearances/expert witness fees: $350/hour, 6 hour minimum.
​
Provider may be contacted during office hours which at this time are considered Monday-Thursday, 9 am-1 pm. Any messages received either by phone, text or email will be responded to that day as possible, but may be within a 24 hour period. There is no call/coverage during non-office hours, weekends or holidays.
​
If you or your child are in crisis or needs emergency care at any time, please call 911 or go to the nearest Emergency Department immediately.
​
Any errors on prescriptions will be rectified as soon as possible with no charge to patient.
​
Phone calls are extremely difficult logistically to coordinate with other busy providers and are frequently hit and miss with valuable information for me to treat my client. Communication with other providers is considered a billable service if the duration of the call is more than 15 minutes and is subject to a $300/hr reimbursement rate.
​
Texting and Emails are a preferred way to communicate as they are quick, easy and to the point. Dr. Hall uses iPlum for texting and phone which is HIPAA compliant.
​
Many school or camp forms are quick and simple to complete and are not generally charged for if completed during an office visit and/or take less than 5-10 minutes of my time. Preparation of more substantial reports and letters are not “free” and subject to a $300/hr fee Most letters or reports that I generate are considered medico-legal documents which are taken seriously and thus billed accordingly.
Therapy is recommended for all clients but is not mandated. Dr. Hall specializes primarily in medication management, but also provides talk therapy for those who request it.

Non-participation or non-payment are grounds for termination. Patients can certainly leave the practice for any reason at any time but are responsible for closing any existing balance on their account.
